CSF Surgery only has surgeon specialists.
We don’t have primary doctors or internist.
The process is direct and simple!

- Download our referrals form, fill it out and send it via fax or email with the patient’s information to 661.327.7255 or referrals@csfsurgery.com
*Please make sure to attach the patient’s medical diagnosis, exam results, images, and laboratories.
- Our highly trained bilingual patient advocates will contact the patient and explain the process with CSF. If the patient agrees, an appointment will the CSF specialist will be scheduled.
- Our patient advocates will fax a case update to the referring facility.
- After the appointment with the specialist, if the patient needs surgery, CSF Surgery will create a reduce cost surgical packet for the patient.
- If the patient agrees, the surgery will be schedule.
- CSF will notify the referring facility of the surgery. If the referring facility need additional information, they must contact the surgeon specialist office. For more information call us at 661.333.5746.
With CSF Surgery the referral process is direct!
Our Patient Advocates handle each case from start to finish. After the patient completes their surgery, the Patient Advocates follow-up with them.
